For individuals with a medical condition that warrants monitoring, Metro Home Health Care offers our patients the peace of mind of telemonitoring. This allows us to stay connected with our patients, providing the support and responsiveness they need on a daily basis.
To provide this enhanced level of care, Metro Home Health Care utilizes new innovative technology in patients’ homes. It is an accurate, easy to use patient monitoring system. With our monitoring system, registerd nurses have immediate access to updated patient information.
First thing each morning the patient weighs himself and checks his blood pressure using a simple automatic machine and a special scale. The patient then presses send on the transmitter provided and the information is sent over the phone line to a secure computer monitored by a Metro Home Health Care registered nurse. If any readings are elevated the nurse will call and assess the patient by asking questions about how they feel at that time. Based on that assessment either a nursing visit will be made or the physician will be notified to possibly adjust the medications. By promptly reacting to a potential problem hospitalization can be avoided.
