Metro Home Health Care’s highly trained and experienced clinician team works closely with physicians to develop a personalized treatment plan. Together, we ensure that patients receive the services and care the patient needs for the best clinical outcomes.
We offer physicians 24/7 on call support either by phone or via accessing and completing the on line referral form by double clicking Physician Referral Form.
To insure seamless care of the patient, Metro Home Health Care also offers physicians a customized, secure access to pertinent information regarding patients via our Physician Portal (Physician Access). Please contact us to explore how Metro’s Physician Portal offers the physician added value and support.
To encourage greater involvement of the physician in a patient’s home health care services, starting April 1, 2010, the patient must have a face-to-face meeting with the physician certifying home health care services within a certain period of time. In order to meet this requirement, Metro Home Health Care is making available to all referring physician the face-to-face form. Please double click, complete and return the referral/face-to-face form.
We offer a full range of nursing and therapy options with year-round service, and an on-call Supervisor R.N. for patient and physician consultation after 5 p.m., Monday thru Friday and all day on weekends and holidays.
